Care Navigation
You may notice over the next few months that our receptionists start to ask you why you need to see a GP. This is to help us in our new approach using Care Navigation. Most GP surgeries in Sheffield will be doing this. All of our receptionists have undergone training to help in assisting you to the most appropriate service. Our receptionists will not be giving clinical advice or triaging, they will however be able be offering other services which may be more suitable for your problem.
Some of the services reception may suggest are:
Pharmacy minor ailment scheme
PEARS opticians (acute Eye problems)
Community support worker
IAPT (Improving access to Psychological therapies)
Health visitors
District nursing team
