FAIR PROCESSING NOTICE
This practice is taking part in the YORKSURe research study run by King’s College London. As part of the study, men aged 65-79 years may receive a letter and a urine self-testing kit in the post. Full details about how your data will be used will be included in the kit information if you are selected to take part.

If you are identified as eligible to receive a self-testing kit in the post, your contact details will be shared with a mailout company who will send out the kits. You will not be included in this if you have; • Already opted out of your GP sharing your data • Ever had a diagnosis of bladder or kidney cancer • Had a diagnosis with any other cancer (except non-melanoma skin cancer) within the past 5 years • Been diagnosed with dementia, Alzheimer’s, or Parkinson’s
