Manchester Road Surgery is sited at the corner of Stephen Hill Road and Manchester Road, adjacent to the shopping area of Crosspool. The surgery can be easily reached by using public transport. Bus service 51 stops on Sandygate Rd opposite the post office and is a 2 minute walk to the surgery. Additionally Bus service numbers 244, 273, 274 and 275 run along Manchester Road towards Rivelin and return to the city centre. View surgery on Google map. We do not have a car park on site, however you can usually secure on street parking close to the surgery.
If you live within the practice area and wish to register with our practice you can download the form you need here. Please complete the form, sign it and either post it to us or drop it in at the surgery. Alternatively call in at the surgery and ask the receptionist for a registration form or contact our reception by telephone and the form/s will be posted to you. Please note that you will need one form for each member of the family. You have the right to express a preference to receive services from a particular doctor and this preference will be documented in your medical records.

Please inform us if you have a mobile telephone and are happy to be contacted on this number. As well as being useful should we need to contact you urgently, we are now able to offer appointment reminders and notify you of flu clinics etc. Also please inform us of any change of address or telephone number as soon as possible. This will help us to avoid any unnecessary confusion.
