Klinik Online Triage

We operate a total triage system at Manchester Road Surgery. All requests must go through our online system, Klinik. If you cannot access it via the internet a receptionist will be happy to go through the form with you over the phone.

Same day appointments are intended for urgent cases, the doctor will assess your request and assign you an appointment accordingly.

Routine appointments

If you cannot use the internet to access this please telephone 0114 266 8265 or call in during opening hours and a receptionist will go through the form with you.

Please record that you have arrived

It is important that you use our self check in screen located in the porch to record that you have arrived.  If a patient arrives after you but is called to see the doctor before you it is probably because they are seeing a different doctor.  If you feel that you have been waiting too long, please speak to the receptionist to make sure that you have not been overlooked.

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

Please try to keep your appointment. If you can’t please tell us in good time to enable the appointment to be given to another patient.

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • phone us on 0114 266 8265, Monday to Friday from 8am
  • reply CANCEL to your appointment reminder text message

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need a chaperone
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.

To request a home visit, it’s helpful if you phone the practice before 11am.

Referrals

When you visit the surgery and are then referred to another service the following information will normally be included in the letter your doctor sends to the hospital or other service provider:

  • Your name, gender, date of birth, home address and NHS Number, where known. If applicable, the name and contact details of your representative.
  • Relevant information about the care and treatment provided to date.
  • Relevant medical history, allergies, prescribed drugs and patient preferences.
  • Infections that need to be managed (if relevant).
  • The reason for the referral and what is required.
  • The name of the person who has made the referral.
