Your comments and suggestions are important to us, if there is anything you think we can improve on in terms of our service we would love to hear form you.

Only use this form for comments about the practice and suggestions as to how we can improve our service to you.

Medical matters and official complaints cannot be dealt with via this form.

Complaints

PLEASE DO NOT USE THE COMPLAINTS FORM IF YOU NEED A MEDICAL APPOINTMENT. PLEASE USE OUR APPOINTMENTS PAGE AND FOLLOW THE GUIDELINES

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

To pursue a complaint either...

  1. Fill out our online complaints form
  2. Give our reception team a call on 0114 266 8265 who will complete a complaints form on your behalf.
  3. Attend in person with your complaint written down with your contact details on.
  4. Download our complaints form and hand it in at reception.

We will acknowledge your complaint within 3 working days. 

 

GP patient survey

The GP Patient Survey is an independent survey run by Ipsos on behalf of NHS England. The survey is sent out to over two million people across the UK. The results show how people feel about their GP practice.
